Making a referral
If you feel someone may benefit from the services provided by St Ann’s, this page will give you a quick guide to referring them to us.
All health and social care professionals can make a referral to St Ann’s services.
Patients, families and carers can seek a referral directly for The Being You Centre, and also for the Community Specialist Palliative Care Team in Salford (in consultation with relevant health and social care professionals).

Inpatient care
Referrals can be made through a healthcare professional at a hospital, such as a consultant or a doctor, a GP or palliative care specialist nurse who will need to complete and submit our Referral Form (which can be downloaded below, details of the return address can be found on the form itself).

Family members, carers and patients can’t refer directly for our inpatient services. But you can get in touch with our 24hour advice line for more information and support:

The Being You Centre
Referral can be made by GPs, district nurses, hospital medical teams, community or hospital specialist palliative teams and social care teams. To refer someone, download and complete a Referral Form below, details of the return address can be found on the form itself.
You can also refer yourself to the Being You Centre, with your GP’s help or directly to St Ann’s.

Community support
Hospice@Home teams (Salford and Trafford)
Patients and/or carers can ask their doctor, district nurse or specialist palliative care nurse to contact the team on 0161 702 5405.
Community specialist palliative care nurse team (Salford)
Referrals can be made by any health and social care professional and can also be initiated by the patient, family or carer (in consultation with relevant health and social care professionals). Referrals can be discussed with any member of the community specialist palliative care team on 0161 702 5406

Homeless palliative care service
Referrals can be made by any health and social care professional, including support workers and hostel staff. Download the referral form below and email the completed form to nehgm.admissions-hg@nhs.net
